What I Do
I provide nutritional therapy to clients to assist them in improving their health and wellbeing through diet and lifestyle advice. Through naturopathic nutritional therapy consultations, dietary and lifestyle analysis, and functional biochemical I aim to understand the underlying causes of your health issues and seek to address them through personalized dietary, supplement and lifestyle advice.
Your Personal Data
I may collect the following information provided by you by completion of questionnaires, during nutritional therapy consultations or by email telephone or post:
· Basic details such as name, address, contact information, gender, date of birth
· Details of contact I have had with you such as referrals and appointment requests
· Health information such as previous medical history (and that of your family), diet, lifestyle, supplement and prescription medicine, biochemical test results, blood type, clinic notes and health improvement plans
· GP contact information
I use this information in order to provide you with health advice, therefore the legal basis of my holding your personal data is for legitimate interest.
I may use your information to improve my services. I may also from time to time send you promotional emails about new services, offers or information about Rise Rooted Nutrition that I feel you may find interesting.
I may obtain sensitive medical information in the form of test results from biochemical testing companies or other healthcare providers. The provision of this information is subject to you giving me your consent. I use this information to provide you with health advice, therefore the legal basis of my holding your personal data is for legitimate interest.
Following completion of your nutritional therapy I retain your personal data for the period defined by my professional association the FNTP, NNA and SON registrant bodies. This enables me to process any complaint you make.
How I use your personal data
I act as a data controller for use of your personal data to provide health advice. I also act as a data controller and processor in regard to the processing of your data from third parties such as testing companies and other healthcare providers.
All records held by me will be kept for the duration specified by guidance from my professional association FNTP.
Sharing Of Your Personal Data
I will keep information about you confidential. I only disclose your information to third parties with your express consent with the exception of the following:
· My professional associations FNTP, NNA and SON for any complaint made by you
· Any contractors and advisors that provide a service to me or act as my agents on the understanding that they will keep your information confidential
· Anyone to whom I may transfer my rights and duties under ny agreement I have made with you
· Any legal or crime prevention agencies and/or to satisfy any regulatory request if I have a duty to do so or the law requires me to do so
· I may share your information with supplement companies and biochemical testing companies as part of providing you with direct healthcare.
I seek your consent before sharing your information with your GP or other healthcare providers. However, if I believe your life may be in danger then I may pass your information onto an appropriate authority such as the police, social services or GP (the in the case of self harm) using the legal basis of vital interests.
You have the right to see, amend, delete or have a copy of data held that can identify you, with some exceptions. You do not need to give a reason for your request.

I only use information that may identify you in accordance with GDPR. This requires me to process personal data only if there is a legitimate interest for doing so and that any processing may be fair and lawful. I ensure the information I hold is kept in secure locations and restrict access to authorized personnel only. Confidential information held electronically is stored on a password-protected computer, in a password-protected electronic folder. Paper documents will be scanned and stored electronically in the same way, and then stored in a locked cabinet at either my home address or the clinic from which I practice.
I am registered with the Information Commissioner’s Office (ICO) as a data controller, a copy of this registration is available through the ICO website (search by business name).
